I understand that the products I have applied for are offered by Western State Bank (the "Bank"). If my application is approved, the accounts I have indicated will be opened by the Bank and will be governed by North Dakota and applicable federal law. I understand that the use of my account(s) opened in connection with this offer will constitute acceptance of and subject me to the terms and conditions of the agreement(s) that govern my account(s). Where permitted by law, I agree to receive my disclosures electronically, and where I chose, I have downloaded or printed them. I understand that the terms of my account(s) are subject to change as provided in the applicable agreements.

This shall serve as my authorization and approval should I choose to initiate an ACH transfer via telephone.

I agree that the Bank has the right to obtain a current consumer report in connection with this application, the renewal of my account, or a change in my credit line; and that the Bank has the right to report to others its credit experience with me. Upon my request, I will be informed whether or not such a report was requested, and if so, the name and address of each agency which furnished this report.

If I choose to submit this application electronically, I acknowledge that, regardless of whether I use a web browser with security features, the Bank is unable to ensure that the data will not be intercepted by third parties. I agree that the Bank will not be liable should such interception occur. The Bank strongly suggests that you use a browser with a 128-bit encryption.
